Is there not an application form for the whitelist?

If not I guess I'll just state what the above user has:

Currently using ModLoaderMp 1.1 v2.1, MinecraftForge v1.3.3.14, and IC2 V1.64

However when launching the server it throws up this error (and has done for ModLoader 1.1 v2 and Forge 1.3.1)
